Notice to Patients Regarding Medical Records Submission

Our providers are committed to delivering the highest quality care to our patients. To ensure comprehensive and effective treatment, we request the following:

New Patients

Please arrange to have your medical records sent to our office ideally two weeks prior to your appointment. This allows our doctors to review your records beforehand and provide you with the best possible care.

Established Patients

If you visit any specialist or undergo procedures such as X-rays, scans, ultrasounds, or surgeries, please fax the records to 877-254-0566. This enables us, as your primary care providers, to maintain a comprehensive understanding of your health.

Medical Records Request Form

Attached is our Medical Records request form. Please complete the form thoroughly, ensuring the following details are clearly provided:

  • Name and phone number of the physician/specialist/facility from which you are requesting records
  • Your name, date of birth, and signature

Specific Information for Patients of Doctor Feeney

  • We have a view-only copy of certain records from Temecula Valley Family Medicine.
  • Unfortunately, we no longer have access to Dr. Boggeln’s records.

We appreciate your cooperation in ensuring that your medical records are up to date and complete. This is crucial for providing you with the highest standard of care. If you have any questions or need further assistance, please do not hesitate to contact our office.

If you have moved or need a copy of your medical records, there is a $25.00 fee for printed copies, regardless of the size of the chart. However, there is no charge to fax or email medical records to a physician or specialist’s office. Please note that a signed medical release form is required for any transfer of records.

For extraction of medical records for Law Offices there will be a $25.00 fee require.

Dr. Feeney’s New Patient Policy

Dr. Feeney has a strict policy for new patients. If Dr. Feeney has never seen you before, you must complete the medical records form provided above and email it to jharris@atigafamilypractice.com or fax it to 877-254-0566.

Dr. Feeney requires that our office receive two years of your medical records within five business days of your scheduled new patient appointment.

If you encounter any difficulties with transferring your medical records or if you have not seen a doctor in the past two years, please fill out Dr. Feeney’s Medical History form attached below in detail.
